how do I know what ring size I need to order refills? I have a universal pocket

Hello. You order your planner pages based on the size of your binder. It's not based on ring size. You said you have a universal pocket, so if your binder is pocket size, you will order pocket size planner pages. If you are unsure of your binder size, our call center representatives can help you determine that. They can be reached at 1-800-654-1776. Our Binder Planner Sizing page may also help.

Do any of you pocket binders have a place for pens? Does the pen fall out if not carried properly?

Hello. All of our binders, including for Pocket, have at least one pen loop for pens. Some have two.
